Job Description & Requirements Job Description: \n\nThe RN Case Manager is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient’s resources and right to self- determination.\n\nShift: Day Shift 5 x 8s; Rotating days; 08:00 to 16:30.\n\n \n\n Required Qualifications:
\nTwo years acute care RN experience required.
\nGraduate of an accredited nursing program.
\nLicense, Certification, Registration
\nRegistered Nurse License (in the state where care is provided)
\nBLS (AHA)
