Job Summary


The medical Social Worker performs advanced clinical evaluations and treatment of persons in need of medical or mental health services in a healthcare setting. Working primarily with pediatrics, but there is an opportunity to transition to the adult team. This role involves major clinical responsibility without restriction to type of problem or severity of medical illness. The work requires independent judgment but operates within defined policies and procedures. This position acts as a consultant and backup to other clinicians in implementing treatment plans and demonstrates expertise in working with specialized disease entities or populations such as oncology patients, AIDS patients, pediatric patients, renal patients, etc. Leadership and training of recent graduates, entry-level social workers, and social work students are key features of this role.


Key Responsibilities

• Prepares psychosocial assessments of patients and families for the interdisciplinary team and participates in treatment and aftercare planning.
• Provides psychosocial counseling to patients and families related to illness or injury and compliance with medical treatment plans.
• Coordinates discharge planning, including referrals to health care service providers and other community supports.
• Participates in interdisciplinary rounds and conferences.
• Provides in-service training to social work staff and other disciplines.
• Trains and provides leadership to social work students and entry-level social workers.
• Completes required statistics and reporting for the Social Work Department.
• Keeps abreast of developments in medical specialization areas.
• May represent the hospital at community meetings and act as a liaison between community and hospital programs.

Qualifications

• Master's degree in Social Work (M.S.W.) from a School of Social Work accredited by the Council on Social Work Education, required.
• Possession of a valid license and current registration as an LMSW or LCSW, issued by the New York State Education Department.
• Must maintain license and registration throughout employment.

Experience

• Minimum of 2 years of experience in professional social work in a medical, mental health, or human service setting as a member of a multidisciplinary treatment team, required.

Skills

• Thorough knowledge of principles and practices of medical social work.
• Understanding of disease entities, medical technology, and treatment modalities.
• Skill in crisis intervention techniques and conducting interviews.
• Ability to work effectively with patients and families facing catastrophic illnesses and injuries.
• Effective communication, both orally and in writing.
• Ability to lead and train subordinate staff.
• Proficiency in computer applications such as spreadsheets, word processing, and email.
