We’re seeking an MDS Coordinat or to help develop individualized care plans and maximize reimbursement — all while upholding the highest standards of care.

Key Responsibilities:

  • Develop care plans based on MDS assessments in collaboration with the interdisciplinary team
  • Ensure the facility is compliant with all regulations and guidelines related to MDS
  • Communicate effectively with staff, families, and healthcare providers to coordinate care
  • Maximize reimbursement by accurately reflecting care needs in the MDS
  • Update MDS assessments regularly to reflect health or condition changes
  • Contribute to overall quality assurance by assuring completeness and accuracy of MDS documents
  • Gather information through direct observation and collaboration with care team members

The Ideal Candidate :

  • Licensed Registered Nurse (RN) required
  • RAC-CT (Resident Assessment Coordinator - Certified) certification required
  • Experience with MDS and care plan development
  • Strong communication and organizational skills
  • Detail-oriented and comfortable navigating regulations
  • Collaborative team player who advocates for resident care
  • Familiar with reimbursement processes and compliance requirements
