You Will: Provide education and help strengthen daily living skills to support engagement in outpatient care and community resources. Provide a variety of case management services, under supervision, assisting clients to apply for several types of public assistance and providing supporting documentation. Attend multi-agency clinical case consultations, and facilitating referrals to other organizations. Support client recovery by developing and enhancing client access to resources for vocational, financial, medical, housing, utilities, food, transportation or basic living needs. Facilitate groups, classes and workshops. Assist other teams with outreach, engagement, and paperwork. Participate in team meetings to staff cases and communicate administrative information.
