Job Description

We are a Home Health Agency in Thousand Oaks, CA that is looking for a self motivated, compassionate RN or LVN Home Health Case Manager! (Non-licensed clinicians/medical staff are encouraged to apply so long as they have previous home health experience)

The job requires the ability to utilize a PC, printer, multi-line telephone, and other related office equipment. We do not offer work remotely.

Responsibilities

Schedule is Monday-Friday 8:30am-5pm.

• Coordinates the patient’s and families care and services including those of the team, social worker, home health aide and physician
• Responds necessary information to the Director of Nursing Monitors the re-certification / re-authorization process
• Ensures and documents appropriate use of the medications, durable medical equipment and supplies
• Educates patients, families and other caregivers Appropriately utilizes the skills and resources of contract personnel such as PT, OT, Podiatrist and Pharmacy
• Provides appropriate care plan and integrates care plans as needed
• Participates in routine chart reviews Attends regularly scheduled in-services and educational conferences.

QUALIFICATIONS

1. Must be LVN/RN. (Non-licensed clinicians/medical staff are encouraged to apply so long as they have previous home health experience)

2. Must have a minimum of 1-year Hospice or Home Health Care experience, (ability to use Axxess Home Health Software)

3. Must possess typing and other clerical skills.

4. Fluent in English/Spanish speaking and writing.

JOB TYPE : Full-time

Salary: Starting at $60,000 per year PLUS Mileage and Bonuses. (*If unlicensed nurse, starting pay is subject to change.)
